This is a full time, direct care position. The primary responsibilities of the Extender are to:
Perform general outreach, engagement, and follow-up with members.
Spanish language proficiency preferred.
Coordinate services/appointments.
Engage clients in health promotion activities.
Share information with Care Manager on the member's circumstances.
Make and track referrals.
Participate in case conferences.
Support the care manager in assessing and addressing unmet health-related resource needs.
Requirements:
Must be a be at least 18 years of age, have a high school diploma (or equivalent)
1-2 years paid experience preferred within behavioral/mental health field.
Spanish speaking/bilingual preferred.
