As part of a person centered care model, this role will provide assistance to the case management team so quality care can be delivered in a timely & efficient manner. Must be highly organized, able to multi-task and have strong computer skills. This role will collaborate with all team members to achieve exceptional outcomes.

Job Duties:

Collaboration with peers across the regions

Process improvement with providers and suppliers

Communication with referral sources, vendors, patients and all team members

EMR updates as necessary

Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases

Consults with physician regarding client needs and plan of care, with pharmacies, DME

Directs patient complaints/issues to appropriate personnel

Participates on agency committees and special projects

Attends and participates in in-service education program

Participates in Team meetings and case conferences, LEAN Daily Management process, including huddles, scorecards, Visual Boards, IDT

Utilizes data management systems to manage patient documentation, calculates and analyzes data from reports to ensure timely & accurate compliance and month close

Maintains knowledge of and compliance with Federal, State and accreditation regulations as well as Agency policies and procedures

Seeks consultation and guidance from supervisor

Maintains productivity standard

Coordinates agency service delivery and solves logistical problems for customers and clients

Provides client information to physician's offices and receives care plan changes via verbal orders

Documents specific patient information including verbal orders from physicians; transports verbal orders from physician's offices as needed

Intervenes via telephone and/or through the application of tele-home care technology for patients; communicates changes in health status of patients to appropriate clinicians

Refers callers to appropriate community resources or agency services when needed

Receives and relays information between field staff, physicians, and other resources

Participates in quality assurance and quality improvement activities

Qualifications

Education/Licensure:

Licensed practical nurse with a license to practice in the state of Connecticut

Experience: Two years of hospital experience
