Summary:
Job Summary: Serves as the interdepartmental liaison between the Case Management team members and Providers to address patient care issues and inquires in an efficient/consistent manner. Provides guidance, orientation, training, and resources to team members. Collaborates with the care team in the provision of patient care. Performs a clinical review of selected patients to determine service needs. Coordinates the care of selected patients. Integrates and coordinates access and utilization management, proactive patient panel management, care facilitation, and treatment planning functions. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Qualifications: Required: Possess one of the following: • Masters of Science Degree in Nursing (MSN) from an accredited institution. • Bachelors of Science Degree in Nursing (BSN) from an accredited institution and 5 years of case management or manager care experience. Possess a current license to practice as a Registered Nurse in the State of Ohio. 3 years of case management or managed care experience. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication, and delegation and problem-solving skills. Demonstrates strong analytical, data management, and computer skills. Demonstrates strong leadership, organizational, and time management skills. Reliable transportation to make clinic or home visits to patients as needed. Preferred: Master's Degree in a healthcare-related field from an accredited institution. Current Certified Case Manager (CCM) by the Commission for Case Manager Certification (CCMC), or other similar case management certification. Experience with case management, access management, home health care, psychiatric case management, discharge planning, or similar related experience. Knowledge of EPIC functionality. Knowledge of case management software. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
