Job Summary

Reentry Services at Trilogy receives referrals from both Illinois Department of Corrections (IDOC) and Cook County Sheriff’s Office (CCSO) to support individuals with Severe Mental Illness who are at risk of homelessness when returning to their communities. The programs offer a mix of permanent supportive housing, short term transitional placement, and high intensity case management services. The program prioritizes individuals with severe mental illness, Murphy Class members (individuals with sex offense convictions), individuals in need of placement for pretrial electronic monitoring, and other individuals considered “hard to house” and otherwise unable to leave a correctional facility without a housing placement.

The Reentry Team Leader will oversee and provide direct supervision to case management team. They will run team meetings, manage capacity and workflows to ensure coverage across our service area and referral systems, and oversee the day-to-day operations of CCSO and IDOC referral programs. The Team Leader will communicate team expectations regarding intake and assessment services, facility discharge planning, access to housing, linkage, coordination, and level of care needs; and will ensure that employees are well-informed about services in the community in order to collaborate with partners to meet the needs of clients.

 

Responsibilities

  • Conduct assessments and treatment planning as needed for newly intaked clients, and support team with identifying priorities and goals based on client needs.
  • Effectively split time between direct service, administrative, and supervisory responsibilities and ensure both clinical and administrative duties are fulfilled.
  • Provide direct clinical supervision and consultation to Housing Case Managers and Recovery Counselors. Orient and train new employees to provide quality services to clients. Ensure continued education and training for assigned employees to strengthen services provided.
  • Provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and justice system involvement, from a restorative justice lens, utilizing a harm reduction approach; and coach staff to do the same.
  • Maintain consistent relationship with referral partners at IDOC and CCSO to ensure seamless referral workflow and ensure optimal program operations.
  • Manage employee workflow; and monitor/adjust employee schedules accordingly to ensure coverage of discharges from IDOC or CCJ, ongoing support with parole / probation / pretrial services, and case coordination and management,
  • Communicate effectively with internal team, other Trilogy teams, and external partners for purposes of consultation and information sharing; e.g. report on openings for the team. Report on trends, client needs, and team updates to Supervisor.
  • Support Clinical Director with grant compliance, including monitoring and auditing staff documentation, and completing monthly and quarterly reports.
  • Ensure timely and seamless flow of new Reentry clients into the organization.
  • Lead clinical documentation process, including IM+CANS, LOCUS, and all transfer documentation.
  • Provide on-call coverage and immediate crisis intervention as needed.
  • Work with Supervisor to comply with expectations of various funding sources.
  • Perform other related duties and/or projects as assigned.

 

Qualifications

  • Must have one of the following:
    • Master’s Degree in Social Work, Psychology, or Counseling
    • Bachelor's degree in relevant field plus 5 years of experience working in housing, case management, and/or reentry services
  • Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) preferred
  • Minimum two years’ experience working with people with mental health and/or substance use diagnoses
  • Leadership experience highly preferred
  • IM+CANS certification preferred
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
  • Experience working with low income housing, housing navigation, or homeless services preferred.
