Telehealth Care Coordinator – Healthcare

GoTo Telemed Services is hiring Telehealth Care Coordinators to support patients through a remote, technology-enabled healthcare environment.

We are seeking individuals with healthcare education, training, or professional experience who are interested in applying their healthcare knowledge in a care coordination and telehealth support role.

This opportunity is open to qualified applicants nationwide, including all 50 U.S. states, subject to applicable employment, professional, and state requirements.

Who Can Apply

• Medical students with healthcare-related training
• Nursing students or healthcare students
• Healthcare graduates
• Registered nurses and other licensed healthcare professionals
• Allied health professionals
• Individuals with relevant healthcare education and patient-support experience

The role is focused on care coordination and patient support, not independent medical practice. Applicants must only perform activities permitted by their education, training, credentials, employer policies, and applicable laws.

Responsibilities

• Communicate with patients through approved telehealth communication channels
• Help patients understand their care journey and next steps
• Coordinate appointments, follow-ups, referrals, and healthcare resources
• Assist with patient intake and collection of relevant information
• Support patient engagement and follow-up
• Help patients navigate available healthcare services
• Document interactions accurately in the designated system
• Escalate clinical concerns to the appropriate licensed healthcare professional
• Follow GoTo Telemed protocols, privacy requirements, and patient-safety procedures
• Provide educational and administrative support within the scope of the role
• Work collaboratively with providers and the telehealth care team

What You Will Not Do

This is not an independent clinical provider position for applicants who are not appropriately licensed and authorized.

Care Coordinators do not independently diagnose medical conditions, prescribe medications, order treatment, or represent themselves as a licensed healthcare provider when they are not authorized to do so.

Qualifications

• Current enrollment, education, training, certification, or professional experience in healthcare
• Strong communication and interpersonal skills
• Ability to communicate professionally with patients
• Comfortable using computers and telehealth technology
• Strong organization and follow-up skills
• Ability to maintain confidentiality and handle sensitive patient information appropriately
• Reliable internet connection and suitable environment for remote work
• Ability to work independently while following established protocols

Healthcare licensure or certification is a plus but is not automatically required for every Care Coordinator position, provided the applicant meets the applicable requirements for the specific role.
