What Youll Do
Provide and document nursing care to patients at home in accordance with care plans
Support patients to maintain, restore, or improve their health status
Collaborate with healthcare providers and community resources to coordinate personalized care
Evaluate patient responses to interventions and update care plans accordingly
Exercise independent judgment within established protocols and policies
Maintain accurate clinical records and communicate effectively with team members
Be flexible with work hours and willing to accept on-call responsibilities
What You Need
Valid MA LPN or LVN license (must upload license with profile)
Ability to work independently with critical thinking skills
Strong interpersonal and communication skills
Reliable transportation and willingness to drive in various weather conditions
Dependability in meeting work schedule demands
Willingness to accept a 5-day maximum for time-off requests during assignment
