What You’ll Do
  • Observe residents on an ongoing basis and report changes in mental and physical well-being, including when physician notification is needed.
  • Receive, transcribe, and carry out physician orders accurately, including documentation through computerized MARs/TARs and electronic records.
  • Administer medications and treatments as ordered; maintain required pharmaceuticals for the unit.
  • Supervise Licensed Nursing Assistants (LNAs) in accordance with licensing requirements, including assigning and overseeing resident care within scope of practice.
  • Participate in resident rounds and support communication during care reviews (including 60-day review processes).
  • Support resident care activities including oxygen administration, gastrostomy/jejunostomy feedings per policy, and wound care (including wound vac awareness/knowledge).
  • Assist with admissions, discharges, transfers, and required unit reports/shift communications; maintain infection control awareness and a clean, hazard-free unit.

What You Need
Required
  • Current LVN/LPN license
  • Ability to provide skilled nursing care in accordance with physician orders and facility procedures
  • Reliable transportation (required due to the rural location and limited housing/public transit)

Preferred
  • Experience in Long Term Care (LTC)
  • Familiarity with MAR/TAR documentation and computerized charting systems
  • Experience supervising or working closely with nursing assistants/LNAs
