What Youll Do
Monitor residents physical and mental well-being on an ongoing basis
Receive, transcribe, and follow physician orders accurately
Administer medications and treatments per physician directives with proper documentation
Supervise and support Long Term Care Assistants (LNAs) in providing resident care
Assist with resident admission, transfer, and discharge processes
Perform wound care, administer oxygen, and assist with feeding procedures
Ensure infection control practices and unit cleanliness
Communicate effectively with residents families and interdisciplinary team members
Maintain compliance with state and federal regulations regarding medication storage and documentation
Support orientation and training of new staff members
Assist in ensuring a safe, hazard-free environment in the facility
What You Need
Required:
Valid LPN or LVN license in New Hampshire
Basic Life Support (BLS) certification
1+ year experience in long-term care preferred
Ability to work independently and as part of a team
Strong attention to detail and communication skills
Preferred:
Experience supervising nursing assistants
Knowledge of electronic health records (EHR) systems
