What Youll Do:
Monitor and observe residents physical and mental well-being, reporting changes promptly
Administer medications and treatments as prescribed; accurately document on electronic MARs/TARs
Supervise and support nursing assistants, ensuring quality resident care
Assist with resident rounds alongside physicians and communicate medical needs effectively
Perform wound care, administer oxygen and enteral feedings following facility protocols
Manage admissions, discharges, and transfers with proper documentation
Communicate with residents families regarding care updates and concerns
Manage unit responsibilities, including infection control, safety checks, and documentation
Follow all state and federal regulations concerning medication storage and safety
Support new staff orientation and ongoing education as needed
What You Need:
Valid LPN/LVN license in New Hampshire
Minimum of 1 year long-term care experience preferred
Excellent communication and organizational skills
Ability to work independently and as part of a team
Reliable transportation (required, due to location)
