What You’ll Do:
  • Monitor and observe residents’ physical and mental well-being, reporting changes promptly
  • Administer medications and treatments as prescribed; accurately document on electronic MARs/TARs
  • Supervise and support nursing assistants, ensuring quality resident care
  • Assist with resident rounds alongside physicians and communicate medical needs effectively
  • Perform wound care, administer oxygen and enteral feedings following facility protocols
  • Manage admissions, discharges, and transfers with proper documentation
  • Communicate with residents’ families regarding care updates and concerns
  • Manage unit responsibilities, including infection control, safety checks, and documentation
  • Follow all state and federal regulations concerning medication storage and safety
  • Support new staff orientation and ongoing education as needed

What You Need:
  • Valid LPN/LVN license in New Hampshire
  • Minimum of 1 year long-term care experience preferred
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team
  • Reliable transportation (required, due to location)
