What You’ll Do:

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
• Develop and implement individualized care plans adhering to regulatory standards
• Provide rehabilitative, skilled, and medically necessary treatment interventions
• Perform specific procedures such as ADL retraining, therapeutic exercises, use of assistive devices, and modalities
• Monitor patient progress and modify treatments as needed
• Document all services accurately in electronic health records
• Collaborate with the healthcare team and communicate patient progress
• Supervise and support occupational therapy assistants, aides, or students when required
• Participate in team meetings, trainings, and compliance activities

What You Need:

• Valid Oklahoma state license as an Occupational Therapist
• NBCOT certification
• Minimum 1 year of relevant experience in LTC or related setting (preferred)
• Strong communication and teamwork skills
• Ability to work independently and manage patient caseload effectively
