What Youll Do
Implement individualized treatment plans to improve patients' physical, social, and mental functions
Conduct patient evaluations to ensure quality standards and identify areas needing attention
Use appropriate clinical techniques and tools for therapy interventions
Communicate care and discharge plans with patients and team members
Complete accurate and timely clinical documentation
Supervise assigned assistants and ensure compliance with facility protocols
Collaborate with interdisciplinary teams to optimize patient outcomes
What You Need
Current Hawaii State Physical Therapist license
Minimum 1 year of home health therapy experience
Strong clinical skills and ability to work independently
Excellent communication and teamwork abilities
