What You’ll Do:

• Evaluate the physical therapy needs of assigned patients in the home health setting
• Develop and implement personalized treatment plans to improve patient mobility & independence
• Document patient care activities accurately and timely in electronic medical records
• Assist patients with transfers and positioning as needed
• Collaborate with healthcare teams to ensure coordinated care
• Provide age-appropriate care to a diverse patient population including pediatric through geriatric

What You Need:

• Valid Illinois Physical Therapist license
• Ability to transfer and position patients safely
• Strong knowledge of physical therapy techniques and documentation standards
• Excellent communication and patient interaction skills
• Prior experience in home health or outpatient care preferred
