What You’ll Do:
  • Screen, evaluate, and develop treatment plans for patients with physical disabilities and wounds in a long-term care setting.
  • Utilize standardized assessments and evidence-based practices to guide interventions.
  • Administer therapeutic exercises, manual techniques, modalities, and wound care procedures.
  • Adjust treatment plans based on patient progress and evaluation outcomes.
  • Communicate regularly with the healthcare team regarding patient status and care plans.
  • Document all treatment sessions and patient progress accurately in electronic health records.
  • Support supervision of rehab aides, PT assistants, or students as needed.
  • Participate in team meetings and ongoing training opportunities.


What You Need:
  • Valid State Physical Therapist license
  • 2+ years of clinical experience, preferably in outpatient, rehab, or LTC settings
  • Strong communication and assessment skills
  • Ability to work independently and collaboratively
  • Knowledge of regulatory and clinical documentation standards

Preferred:
  • Experience with wound care and therapeutic modalities
