What Youll Do:
- Screen, evaluate, and develop treatment plans for patients with physical disabilities and wounds in a long-term care setting.
- Utilize standardized assessments and evidence-based practices to guide interventions.
- Administer therapeutic exercises, manual techniques, modalities, and wound care procedures.
- Adjust treatment plans based on patient progress and evaluation outcomes.
- Communicate regularly with the healthcare team regarding patient status and care plans.
- Document all treatment sessions and patient progress accurately in electronic health records.
- Support supervision of rehab aides, PT assistants, or students as needed.
- Participate in team meetings and ongoing training opportunities.
What You Need:
- Valid State Physical Therapist license
- 2+ years of clinical experience, preferably in outpatient, rehab, or LTC settings
- Strong communication and assessment skills
- Ability to work independently and collaboratively
- Knowledge of regulatory and clinical documentation standards
Preferred:
- Experience with wound care and therapeutic modalities
