Required Skills:
  • Valid state licensure as a Physical Therapist (PT)
  • Proven experience in rehabilitation, wound care, and therapeutic interventions
  • Strong knowledge of standardized assessments, evidence-based practices, and clinical standards
  • Excellent communication skills, with the ability to collaborate effectively with patients, families, and healthcare teams
  • Ability to develop and implement individualized care plans and monitor patient progress
  • Proficient in documenting clinical information accurately in electronic health records
  • Ability to supervise and support physical therapist assistants, rehabilitation aides, and students

Nice to Have Skills:
  • Experience working in Long Term Care settings
  • Familiarity with DME procurement and supply management
  • Additional certifications in wound care, manual therapy, or related specialties
  • Bilingual proficiency is a plus

Preferred Education and Experience:
  • Bachelor’s or Master's degree in Physical Therapy from an accredited program
  • Prior outpatient or LTC experience is highly valued
  • Demonstrated success in inpatient or outpatient rehabilitation services

Other Requirements:
  • Must obtain and maintain all relevant licenses and certifications
  • Ability to travel to associated facilities or locations if required
  • Commitment to ethical practice and adherence to APTA and state regulatory standards
