What You’ll Do

• Evaluate and develop individualized treatment plans for patients with physical functioning disorders and wound care needs
• Implement therapeutic procedures such as manual techniques, ambulation, exercises, modalities, and assistive device use
• Monitor patient progress, adjust treatments, and document outcomes in compliance with regulatory standards
• Provide counseling and education to patients, families, and caregivers
• Communicate effectively with team members regarding patient care and progress
• Assist with discharge planning and prepare comprehensive summaries
• Supervise and support rehab aides, assistants, and students as needed
• Participate in meetings, trainings, and necessary documentation protocols

What You Need

• Valid Physical Therapist license in MA
• Prior experience in long-term care or rehabilitation settings preferred
• Strong communication, assessment, and documentation skills
• Ability to work independently and as part of a team
• Knowledge of regulatory and clinical best practices
