Physical Therapist - Long Term Care (LTC) – Moore, OK

What You’ll Do:
  • Assess and evaluate patients’ physical functioning and develop individualized care plans.
  • Administer therapeutic interventions, including manual techniques, ambulation, and wound care.
  • Monitor patient progress, adjust treatments, and document outcomes in compliance with regulations.
  • Educate and counsel patients, families, and colleagues on treatment options and rehabilitation goals.
  • Collaborate with interdisciplinary teams to ensure quality patient care.
  • Supervise and support rehabilitation aides/students when applicable.
  • Maintain accurate documentation and participate in team meetings, trainings, and conferences.

What You Need:
  • Valid Physical Therapist license in Oklahoma
  • Minimum 1 year of relevant clinical experience
  • Knowledge of APTA standards and state practice acts
  • Strong communication and patient care skills
