Description: The Center for Cardiovascular Health at Brattleboro Memorial Hospital offers a single point of service for patients in need of care during early or advanced stages of heart disease and vascular disease. The Center has one board-certified cardiologist, and two nurse practitioners, and a highly qualified professional staff to provide patients with superior service, from diagnosis to post-procedure recovery. Along the way, we provide our cardiovascular patients with the resources they need to make well-informed decisions about their medical care and treatment.

In this role, you will provide specialized care and support to patients recovering from cardiac events, helping them to regain their strength and improve their cardiovascular health. You will work collaboratively with a multidisciplinary team to develop individualized rehabilitation plans and educate patients on lifestyle changes that promote heart health. This includes but is not limited to:
  • Providing quality triage and
    patient care, utilizing strong assessment skills and point of care testing while
    practicing within the scope of practice defined by the State Board of Nursing.
  • Collaborates with clinicians to assess and establish plan of care for complex
    patients including setting priorities with the patient/family.
  • Demonstrates the
    ability to triage and coordinate patient care with a proactive approach to both
    acute and chronic disease management while being an advocate for the patient s
    rights, value and belief systems.
  • Deliver comprehensive nursing care in a high-volume cardiology
    clinic, performing detailed cardiovascular assessments and triaging
    patients with acute and chronic cardiac conditions.
  • Manage patients with
    cardiac implantable electronic devices (CIEDs), including pacemakers,
    ICDs, and CRT devices.
  • Conducts in-clinic and
    remote device interrogations, evaluating battery status, lead integrity,
    and arrhythmia events. Collaborates with providers to optimize device
    programming.
  • Assist with pre- and
    post-procedure care for device implantation, including patient
    preparation, wound care assessment, and complication monitoring.
  • Administered and managed
    cardiac medications such as anticoagulants, beta-blockers,
    antiarrhythmics, and diuretics; provided patient education on adherence
    and side effects.
  • Educate patients and
    families on cardiovascular disease management, device function, activity
    restrictions, and lifestyle modifications to improve outcomes.
  • Ensures adherence to clinical protocols, safety standards, and regulatory requirements
    in both in-person and remote monitoring workflows.
