What You’ll Do

• Manage healthcare needs and coordinate services for patients within Berkshire Medical Center.
• Review patient admission cases and monitor care throughout hospitalization.
• Perform transition planning for post-hospital care arrangements.
• Collaborate with multidisciplinary teams to ensure quality and cost-effective outcomes.
• Engage in performance improvement initiatives.
• Analyze and present patient data for continuous care enhancement.
• Work independently while maintaining strong relationships with physicians and community resources.

What You Need
Required:

• Massachusetts RN license and Temporary Nursing Service Agency license
• Minimum of proven case management experience
• Strong organizational and communication skills
• Knowledge of Medicare, Medicaid, and third-party payers
• Computer literacy and data management skills

Preferred:

• Experience with hospital case management and discharge planning
• Familiarity with local community resources
