What You’ll Do:

• Provide direct patient care through assessments, planning, and care delivery in a home health setting
• Conduct comprehensive initial and ongoing assessments, including OASIS data collection
• Develop, implement, and modify personalized care plans in collaboration with patients, families, and healthcare teams
• Educate patients and caregivers on health management and safety practices
• Ensure documentation complies with federal, state, and agency regulations
• Promote a safe, clean, and confidential environment of care
• Collaborate with other healthcare professionals to coordinate quality care

What You Need:

• Valid Illinois RN license
• Minimum of 2 years of home health or related nursing experience
• Ability to work independently in patients’ homes
• Strong communication and organizational skills
• Commitment to patient-centered care and safety
