Job Description
- Prescribes coordinates and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other health care workers as assigned.
- Provides comprehensive initial assessments to all clients and on subsequent visits continually assesses Physical, Psychosocial, Environmental and community resources available utilizing the principles of growth and development over the life span
- Initiates Plan of Care, re-evaluates the need for care and modifies Plan of Care as ordered / needed. Establishes discharge plans with patient, physician, other disciplines and caregivers.
- Completes OASIS data
- Provides patient care using current Federal, State and Agency regulations
- Assesses educational needs of patient, families, caregivers and implements appropriate teaching strategies
- Completes documentation according to regulatory guidelines.
- Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
- Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct
Requirement description :
- Home Health experience - [REQUIRED]
- Active BLS or willig to obtain - [REQUIRED]
- Valid drivers license and car insurance - [REQUIRED]
- Copy of agency's IL nursing license included in submission - [REQUIRED]
- Will consider someone with strong Med Surg experience with some Home Health experience

- OASIS and start of care experience - [Strongly preferred]
