What You’ll Do

• Prescribe, coordinate, and deliver patient care through the nursing process
• Perform comprehensive initial assessments and ongoing evaluations of patients’ physical, psychosocial, and environmental needs
• Develop, implement, and modify Plans of Care in collaboration with physicians and other healthcare professionals
• Complete and document OASIS data accurately to ensure quality care and regulatory compliance
• Educate patients, families, and caregivers to promote health and safety
• Maintain a safe, clean, and respectful environment in accordance with standards and regulations
• Supervise and coordinate care with Certified Nursing Assistants and other team members when applicable

What You Need
Required:

• Valid RN license in Illinois or state reciprocity
• Current BLS certification
• At least 1 year of nursing experience, preferably in home health or outpatient settings

Preferred:

• Prior experience with OASIS documentation
• Strong assessment and patient education skills
• Ability to work independently and as part of a team
