What Youll Do:
- Assess and manage pain & symptoms, administering medications to ensure patient comfort
- Develop and update individualized care plans in collaboration with the care team
- Educate and support patient families regarding disease progression and medication management
- Respond promptly to changes in patient conditions, providing crisis intervention as needed
- Document patient care accurately and communicate effectively with the multidisciplinary team
- Offer emotional and spiritual support, including grief counseling to families
What You Need:
Required:
- Valid RN license in Michigan
- Previous hospice or palliative care experience preferred
- Strong communication and compassionate care skills
Preferred:
- Ability to work independently and as part of a team
- Experience in a fast-paced healthcare setting
