Licensed Social Worker (LSW) – Hospital | Days | Minnesota
We are seeking an experienced Licensed Social Worker (LSW) for a hospital-based contract opportunity in Minnesota.
Responsibilities:
- Provide psychosocial support to patients and families during illness, injury, and hospitalization.
- Assist with discharge planning and coordination of care with appropriate next-level services.
- Assess social, emotional, financial, and family factors impacting patient care and recovery.
- Coordinate referrals to skilled nursing facilities, assisted living, hospice, group homes, and community resources.
- Support patients and families with complex social needs, including mental health, abuse concerns, guardianship, and end-of-life planning.
- Collaborate with the healthcare team to ensure safe and timely patient transitions.
Requirements:
- Minimum 2 years of hospital social work experience.
- Discharge planning experience required.
- Experience working with all patient populations.
- Active Minnesota LSW License (must provide documentation).
- Epic EMR experience required.
- Current skills checklist and supervisory references required.
