What You’ll Do

• Document patient interactions accurately and confidentially
• Support discharge planning and post-hospital follow-up

What You Need
Required:

• Valid LMSW license
• Hospital social work experience or equivalent
• Strong communication and interpersonal skills

Preferred:

• Experience with chronic illness management and patient advocacy
• Ability to work collaboratively in a fast-paced environment
