What You’ll Do
  • Conduct screenings, evaluations, and therapy for patients with communication, cognitive, or swallowing disorders in a Long Term Care setting
  • Develop and implement individualized care plans aligned with clinical and regulatory standards
  • Use standardized assessments and evidence-based treatments to support patient progress
  • Monitor and document patient progress and adjust treatments as needed
  • Provide counseling and education to patients, families, and caregivers
  • Collaborate with multidisciplinary team members to ensure comprehensive patient care
  • Complete discharge summaries and maintain detailed, compliant documentation

What You Need
Required:
  • Active ASHA certification and state Speech-Language Pathologist license
  • Prior experience in Long Term Care or similar healthcare environment preferred
  • Strong communication and collaboration skills

Preferred:
  • Experience supervising CFYs or student SLPs
