What Youll Do
- Conduct screenings, evaluations, and therapy for patients with communication, cognitive, or swallowing disorders in a Long Term Care setting
- Develop and implement individualized care plans aligned with clinical and regulatory standards
- Use standardized assessments and evidence-based treatments to support patient progress
- Monitor and document patient progress and adjust treatments as needed
- Provide counseling and education to patients, families, and caregivers
- Collaborate with multidisciplinary team members to ensure comprehensive patient care
- Complete discharge summaries and maintain detailed, compliant documentation
What You Need
Required:
- Active ASHA certification and state Speech-Language Pathologist license
- Prior experience in Long Term Care or similar healthcare environment preferred
- Strong communication and collaboration skills
Preferred:
- Experience supervising CFYs or student SLPs
