Radius rule: will accept locals within 50 miles; 10% reduction to bill rate
Employee Separation Policy: Any past employee of UPH whether FTE, or PRN for the UPH float pool, cannot work travel in another Market no matter which facility they currently work at, or have worked at within the past 12 months. If they worked travel at UPH (not FTE or PRN), the separation policy does not apply
HOLIDAY update: unless the Unit is closed, your contractor is required to work 1-2 of 3 upcoming Summer holidays (Memorial Day, Independence Day, Labor Day)
- NBO: 16 hrs
- EMR: EPIC
- OT/Holiday: 1.15x multiplier
- Call Back: 1.5x multiplier with 2 hr CB minimum
- On Call: ***/hr
- Charge/Precept: ***/hr
Billable OT:
- 36-40 hrs/week, OT billable after 40
- 48 hrs/week, OT billable after 48
Will be required to float across all Des Moines campuses to like units.

- 24 months minimum experience in Cath [required]
- ACTIVE ARRT, or CCI/RCIS [required]
- structural heart experience [highly preferred]
- Interventional Radiology [preferred]
- EP experience [preferred]
- travel experience within previous 18 months [required]
- MUST be able to scrub independently, and proficiently [highly preferred]
- willingness to take call
- must be willing to float across campuses if needed (minutes within one another)
- should have peripheral vascular experience [preferred]
PROFILE REQUIREMENTS:
- Work history/resume/employment history must have 24 months of current (Cath Lab / Cardiac Cath Lab) experience [required]
- Skills Checklist must be specific to the discipline listed on the job (Cath Lab / Cardiac Cath Lab) [required]
- Professional Reference/Reference Check - charge/supervisor/nurse manager/director/CNO/COO performance evaluation/reference from within 24 months of today's date [preferred]
