Job Description

As part of the social services team, works together with the building management to ensure the client's social service needs are addressed appropriately. Responsible for helping clients adjust to independent living through conducting risk/needs assessments, developing and implementing service plans, monitoring and documenting clients progress, and providing support counseling for clients assigned to their caseload.

Scope of Duties:

  1. Conduct initial interview of applicants applying for housing at the building/program.
  2. Inform applicants of paperwork needed to apply for housing at that building/program.
  3. Orientation and assessment of all new clients assigned to caseload via approved agency assessment tools.
  4. Monitor and document all clients' progress according to funding source standards and/or the agency policy and procedures.
  5. Update service plan with client's input.
  6. Schedule and facilitate case management meetings with clients as needed.
  7. Identify and/or develop resources and link clients with needed services.
  8. Develop, implement, or facilitate scheduled groups/programs/workshops addressing client's needs.
  9. Participate in required internal/external treatment meetings.
  10. Provide support counseling to residents.
  11. Conduct apartment/room inspections as needed.
  12. Assist in the development of buildings Tenant Organization (if applicable).
  13. Facilitate Tenant Association/Committee meetings as needed (if applicable).
  14. Complete all required documents and reports in a timely manner.
  15. Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
  16. Demonstrate competency in program related certifications.
  17. Maintain client and staff confidentially in compliance with operational policy and procedures.
  18. Interact and communicate (verbally and non-verbally) with all clients, staff, and outside contacts with respect and courtesy.
  19. Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
  20. Assist the Housing Specialist with moving new clients in approved units.
  21. Annually update client’s leases and re-assessment of income and ability to meet program requirements.

ADA Essential Functions:

  1. Regularly required to stand and walk.
  2. Occasionally lift and move up to 20 lbs.
  3. Ability to observe an area that can be seen up and down or to the left and right while eyes remain fixed on a given point.
  4. Frequently required to use hands to handle and/or feel writing, operating computer and other office equipment.
  5. Occasionally required reaching, bending, and stooping and smell.
