    Full‑Time | Mon-Fri 8am-5pm

    Licensed Clinical Counselor (LPC/LCMHC)

    The Therapist III is responsible for making treatment and level of care decisions independently. This position serves as the primary point of contact for patients enrolled in Collaborative Care, providing clinical assessment, care coordination, patient engagement, treatment planning, symptom monitoring, and communication among the primary care provider, psychiatric consultant, and other members of the healthcare team. The role requires advanced clinical expertise to manage patients with complex behavioral health conditions, develop therapeutic relationships, support treatment adherence, and ensure high-quality outcomes through population-based care management.

    What You'll Do

    Conduct comprehensive behavioral health assessments and screenings.

    • Develop and implement patient-centered care plans using evidence-based practices.
    • Provide brief therapeutic interventions and counseling within the Collaborative Care framework.
    • Monitor patient progress utilizing standardized measurement tools and symptom tracking.
    • Maintain an active patient registry and manage a population of patients with behavioral health needs.
    • Coordinate care between primary care providers, psychiatric consultants, community resources, and other healthcare professionals.
    • Present patient cases during psychiatric case review meetings and implement treatment recommendations

    Identify changes in patient status and facilitate appropriate treatment modifications.

    • Educate patients and families regarding diagnoses, treatment options, wellness strategies, and self-management techniques.
    • Support medication adherence and follow-up activities as directed by treating providers.
    • Manage patients with moderate to high complexity behavioral health needs, including depression, anxiety, trauma-related disorders, substance use concerns, and other mental health conditions.
    • Ensure timely documentation, regulatory compliance, and quality metric reporting.
    • Participate in quality improvement initiatives and program development activities.
    • Provide crisis assessment and appropriate intervention when needed.
    • Assist with achieving organizational goals related to access, engagement, clinical outcomes, and patient satisfaction.

    What We're Looking For

    • Education: Graduate Degree, required. Master's Degree in Psychology, Social Work, Counseling or related field.

    • Experience: Minimum of three years experience, required.

    • Licensure/Certification:

    • Licensed Clinical Mental Health Clinician, Licensed Clinical Social Worker, Licensed Marriage and Family Counselor or Clinic Phd/PsyD level Psychologist, required.

    • For the Substance Use Therapist role, Licensed Clinical Addiction Specialist or Licensed Clinical Addiction Specialist Associate with above required license, preferred.

    • NC state approved curricula for restrictive interventions within 3 months of hire. Team members in strictly remote positions are exempt. Team members working strictly in behavioral health outpatient clinics are exempt from state approved curricula but are required to take NEC0170 De-escalation Techniques and Effective Communication Strategies within 3 months of hire, required.

    • Additional Skills (required): Good working knowledge of computers. Ability to care for Behavioral Health Age Specific populations.
