Overview

Job Summary Evaluates and assesses patients to establish functional measurable goals to promote restoration of functional abilities. Provides treatment according to a written plan of care to meet established goals. Plans for discharge and communicates findings and progress according to professional licensurecertification scope of practice and departmentaldiscipline policies and procedures. Produces other reports as needed. May supervise clinical students and support staff including certifiedlicensed assistants technicians and volunteers. Provides a caring relationship that facilitates health and healing. Provides leadership support participates on hospital committees serves as a resource and ambassador for performance improvement to clinical and management staff. Performs other duties as assigned.

Responsibilities Evaluates and assesses the patient based on subjective and objective information.

Develops a written plan of care.

Provides treatment according to established goals on plan of care.

Documents interventionscare in the patients medical record.

Evaluates effectiveness of plan of care. Reassesses patients functional status.

Provides discharge planning information which includes patientfamily education follow-up and referral to community resources.

Contributes to the development of staff and students.

Participates in activities designed to improve health care delivery.

Completes assigned goals.

Specifications

Experience NA Minimum Required Entry level Preferred Desired

Education Minimum Required Graduate of an accredited OT program. Preferred Desired

Training Minimum Required Preferred Desired Advanced training andor certification in area of primary practice.

Special Skills Minimum Required Basic computer literacy skills. Preferred Desired

Licensure Minimum Required OTBLS CERTIFICATION WITHIN 14 DAYS OF HIRE Preferred Desired
