What You’ll Do:
Provide Therapy Services and management services under established plans of care
Treat patients in SNF and/or Outpatient settings as appropriate
Collaborate closely with an interdisciplinary team
Deliver individualized, meaningful patient care
Complete documentation efficiently and accurately
What We’re Looking For:
VT Licensed PT, PTA, OT or OTA
