    Therapy Supervisor - Home Health

    Position Summary

    The Therapy Supervisor is responsible for overseeing the delivery of high-quality physical, occupational, and/or speech therapy services provided to patients in their homes. This position provides clinical leadership, supervision, and support to therapy staff; ensures compliance with applicable home health regulations and agency policies; and promotes patient-centered care, positive clinical outcomes, and quality documentation.

    The Therapy Supervisor works collaboratively with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary team to coordinate effective patient care and support the agency's clinical, quality, and operational goals.

    Essential Duties and Responsibilities

    • Provide clinical supervision, leadership, and support to therapy staff.
    • Oversee the delivery of safe, effective, evidence-based, and patient-centered therapy services in the home setting.
    • Provide quality assurance (QA) review of the therapy portion of patient charts, including therapy evaluations, treatment plans, visit notes, progress notes, reassessments, recertifications, and discharge documentation.
    • Conduct regular therapy chart reviews and clinical audits to assess documentation quality, clinical appropriateness, regulatory compliance, and adherence to physician orders and individualized plans of care.
    • Identify documentation deficiencies, clinical risks, and opportunities for improvement and provide timely feedback and corrective guidance to therapy staff.
    • Review and monitor therapy evaluations, treatment plans, progress notes, reassessments, and discharge documentation for completeness, accuracy, consistency, medical necessity, and compliance.
    • Ensure therapy services are provided in accordance with physician orders, individualized plans of care, patient goals, and applicable Medicare, Medicaid, state, federal, and home health requirements.
    • Monitor the quality, appropriateness, and effectiveness of therapy interventions and patient outcomes.
    • Provide guidance and feedback to therapists regarding clinical practice, documentation standards, productivity, utilization, and patient care.
    • Assist with orientation, onboarding, training, competency assessment, mentoring, and ongoing education of therapy personnel.
    • Monitor patient progress and ensure therapy interventions and visit frequency remain appropriate to the patient's clinical needs and plan of care.
    • Collaborate with the interdisciplinary team to address changes in patient condition, barriers to progress, and continuity of care.
    • Participate in care conferences, quality assurance and performance improvement (QAPI) activities, agency meetings, and other clinical initiatives as appropriate.
    • Track and trend therapy QA findings and assist with developing and implementing corrective action and performance improvement initiatives.
    • Assist with addressing patient and family concerns related to therapy services and promote timely resolution.
    • Promote compliance with Medicare, Medicaid, state, federal, and other applicable home health regulations and agency policies.
    • Monitor therapy utilization, visit frequency, productivity, and length of stay to support appropriate resource utilization and quality patient outcomes.
    • Support recruitment, retention, scheduling, performance management, and professional development of therapy staff as needed.
    • Assist leadership with the development, review, and implementation of clinical policies, procedures, documentation standards, and quality improvement initiatives.
    • Maintain confidentiality and comply with HIPAA and all applicable privacy requirements.
    • Perform other duties as assigned.

    Qualifications

    • Current, unrestricted professional license appropriate to the discipline and state of practice (PT, OT, SLP, or other applicable therapy discipline).
    • Bachelor's degree required; master's degree preferred.
    • Minimum of 3 years of clinical therapy experience, preferably in home health or a similar community-based setting.
    • Prior supervisory, leadership, or management experience preferred.
    • Strong knowledge of home health regulations, Medicare requirements, clinical documentation, therapy QA, and interdisciplinary care.
    • Excellent communication, organizational, leadership, and problem-solving skills.
    • Ability to work independently and effectively manage multiple priorities.
    • Proficiency with electronic medical records and clinical documentation systems.
    • Valid driver's license, reliable transportation, and ability to travel to patient homes and agency locations as required.

    Preferred Experience

    • Home health therapy experience.
    • Experience with OASIS and Medicare home health requirements.
    • Experience performing therapy-specific QA chart reviews and clinical documentation audits.
    • Experience with quality assurance, performance improvement, and corrective action processes.
    • Experience supervising, mentoring, or educating therapy professionals.
    • Knowledge of regulatory and documentation requirements applicable to home health agencies.
    • Experience monitoring therapy documentation for medical necessity, clinical appropriateness, consistency, and compliance.

    Compensation

    FLSA: Non-Exempt Full-time

    Pay Rate: $60.00 per hour
