What You'll Do
The Thrive Resource Navigator works directly with families participating in Thrive Iowa by providing assessment, support, advocacy, resource navigation, service coordination, care team facilitation, documentation, and follow-up. The Navigator serves as the primary point of contact for assigned families and assists participants in identifying strengths, addressing barriers, accessing resources, completing applications, and improving stability across thirteen Social Determinants of Health domains.
The Thrive Resource Navigator works collaboratively with community providers, schools, health care systems, workforce organizations, child welfare professionals, public assistance systems, and other community partners to help families move from crisis toward long-term stability, family preservation, and self-sufficiency.
The Navigator uses HopeHub with fidelity, completes closed-loop referrals, responds to referrals within required timeframes, completes reassessments every 30 days, and supports families through a strengths-based, family-centered, and culturally responsive approach.
What You'll Need
Preferred: Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Education, Criminal Justice, Public Health, or related field. Required: Associate's degree and relevant experience in human services, family support services, case management, community-based services, or equivalent combination of education and experience as approved by HR. In addition, the Thrive Resource Navigator will demonstrate competency in the following areas:
Technology
Able to use technology that is necessary to perform job responsibilities, including HopeHub, electronic documentation systems, Microsoft Office applications, communication tools, and other systems required by LSI, Iowa HHS, and Restore Hope.
Client Relationships
Be able to demonstrate the ability to build and maintain supportive, strengths-based relationships with clients and their families through interpersonal effectiveness skills, interpersonal sensitivity, techniques, motivational interviewing, and rapport building skills.
Crisis Management
Demonstrate the ability to effectively deal with client and family crisis during provision of service through immediate assessment of emergency, planning, decisive action, community resource coordination, safety planning, and timely follow up to support resolution to crisis.
Trauma Services
Demonstrate the ability to distinguish different types of abuse, the impact of abuse and neglect, and how it can affect development, family dynamics, safety, stability, and engagement with services.
Resource Navigation
Demonstrate knowledge of community resources, public benefit systems, referral processes, family stabilization supports, and social determinants of health. Demonstrate ability to assist families in accessing resources, completing applications, and removing barriers to services.
Knowledge and Understanding of Thrive Iowa and Family Support Services
Ability to utilize appropriate strategies and techniques to provide family support. Demonstrate a comprehensive understanding of services through the successful and practical application of interviewing and assessment skills, knowledge of human growth and behavior, social service systems, crisis intervention, case management, care team coordination, resource navigation, and advocacy with assigned clients.
Documentation and Organization
Maintain accurate records, complete documentation timely, manage multiple priorities, follow up on referrals, track family progress, and ensure HopeHub information is accurate and complete.
Performance Management
Demonstrate ability to use supervision effectively, accept feedback, monitor personal performance expectations, maintain caseload expectations, and meet timelines related to referrals, intakes, assessments, documentation, and follow-up activities.
