Transition Coordinator

TRANSITION COORDINATOR

2024-2025 School Year

RESPONSIBILITIES:

• Coordinate Transition and Post secondary plans for all Special Education students
• Implement, maintain and monitor all state mandated career plans & transition assessments
• Provide student transition instruction
• Liaison to community and state agencies
• Maintain reports for NYSED
• Create and maintain district Special Education Transition program website
• Work collaboratively with district BCBAs and IEP team members
• ABA Data Analysis

REQUIREMENTS:

• NYS certified Special Education Teacher or School Psychologist
• Minimum five (5) years teaching experience
• BCBA certification a plus
• Proof of commencement of NYS fingerprinting
