Transition Specialist

Summary:

• Work Mode: In Person
• Location: Northeast Ohio - Cleveland, Akron, Canton, Ohio
• Shift: Training Mon-Fri 8am-5pm EST; 1-2 weeks for SOAR certification; additional 2 weeks of 1-on-1 training; No time off during training
• Work Schedule: Mon-Fri 8am-5pm EST; 30 min lunch; No overtime; No holidays
• Duration: 6 months, possible extension

Responsibilities:

• Assist members in Northeast Ohio with SSDI applications through in-person visits to gather information, obtain signatures, and deliver applications to local SSA offices.
• Review SNF census, navigate internal systems, and gather medical records according to detailed SOP for SOAR SSA.
• Meet with members and their families to gather pertinent information and communicate effectively, especially with those having language and speech difficulties.
• Navigate SNF environment and collaborate with social workers for assistance.
• Submit required paperwork and follow up timely according to feedback.

Requirements:

• Bachelor's degree or equivalent experience.
• Minimum 2 years of related experience in social work, nursing, or skilled long-term care facility.
• Experience with documentation and member-facing communication skills.
• Ability to get SOAR certified; department will provide training and certification if necessary.

Preferred Skills:

• SOAR certification.
• Experience with SOAR applications.
• ADL (activities of daily living) and EHR clinical knowledge to read and interpret charts.
• Strong report writing and detailed documentation skills.
• Independent work and troubleshooting skills.
• Ability to communicate with diverse populations, including those with speech and language difficulties.
