Job Title: Transition Specialist
Location: Logansport, IN
Duration: 10+months
Type: Contract


Job Description:
  • Facilitate the implementation of a life skills curriculum, titled Next Steps.
  • Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
  • Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
  • Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
  • Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
  • The Transition Coordinators will represent the client- Division of Youth Services at many conferences and meetings.
  • The Transition Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
  • The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
  • Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.
  • Assist students with obtaining social security cards, state identification, driver's license, birth certificates, and additional documentation.
  • Assist students with transitioning back to public schools
  • Assist students with employment, apprenticeship programs, and military service information

Required:
  • Prior working exp. utilizing strong communication skills (written and verbal).
  • Computer knowledge: Microsoft Word, Outlook, and Excel as well as PowerPoint.
  • Prior experience working as a customer service rep.
  • Prior data entry working experience.
  • Prior experience working in a Team environment.
  • Prior exp. working w/at risk youth.
  • Prior working experience in a community or State program.
  • Prior experience working in a youth educational program.
