Overview
As part of the New London Hospital Medical Group, ensures safe and effective patient transitions across the patient
care continuum. As a patient educator, advocate, and patient empowerment facilitator, this position provides
information to the patient and/or caregiver resulting in effective care transitions, improved self-management skills,
and enhanced communications between the patient and the healthcare team.
