Position Summary

The Transitional Housing Case Manager is responsible for providing comprehensive case management services to individuals and families residing in transitional housing programs. The Case Manager assists participants in achieving housing stability, self-sufficiency, and personal goals through assessment, service planning, advocacy, crisis intervention, and coordination of community resources.

Essential Duties and Responsibilities

• Conduct intake assessments and develop individualized service plans.
• Monitor participant progress and provide ongoing case management support.
• Assist participants in obtaining housing, employment, benefits, healthcare, and other services.
• Coordinate referrals and advocate for participants.
• Provide crisis intervention and conflict resolution.
• Maintain HMIS and case file documentation.
• Collaborate with housing providers and community partners.
• Participate in meetings, trainings, and program activities.

Required Qualifications

Bachelor’s degree in social work, Human Services, Psychology, Counseling, Sociology, or related field. Minimum two years of case management experience working with vulnerable populations. Strong organizational, communication, and documentation skills.

Preferred Qualifications

Experience in transitional housing, rapid re-housing, or supportive housing programs. HMIS experience. Knowledge of trauma-informed care, motivational interviewing, and harm reduction. Bilingual English/Spanish preferred.

Working Conditions

Combination of office, field, and community-based work. Local travel may be required. Occasional evening and weekend hours may be necessary.

Salary: $65,000
