Short Description:

Formulates the discharge plan with patient, families and the care team based upon a needs assessment. Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs. Coordinates care authorization process with insurers Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status 

 

 

Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families.

 

    

Formulates the discharge plan with patient, families and the care team based upon a needs assessment.

 

Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community based programs.

 

Coordinates care authorization process with insurers

 

Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status

Experience Required:

3 - 5 years health care experience preferred

Education Requirements:

Requires a Master Degree in Social Work (MSW)/ or BSW or a licensed RN, BSN preferred 

License/Certification Requirements:

Current NJ SW License, LCSW Preferred.

or

Current NJ RN License

 

ACM (American Case Management Association) preffered

CCMC (Commission for Case Manager Certification) preffered

Special Requirements:

Part time, 24 hours a week

Mid shift 

20 hours a week

10a to 6:30p

Salary Min ($): USD $34.00 Salary Max ($): USD $56.00
