Transplant Coordinator III (NP) - Pulmonary Hypertension Cl - Full Time 8 Hour Days (Exempt) (Union)

The Transplant Coordinator III-NP, under the direction of a collaborating physician,assistsin the care of patients with a variety of end stage organ diseases. Coordinates andparticipatesin the medical/surgical management in either inpatient and/or outpatient setting of transplant and/or end stage organ diseases. Performs comprehensive history and physical exams, develop differential diagnoses, implementplanof care and documentpatient'sresponses. Order medications, diagnostic studies, and therapeutic procedures related to their illnesses and trackresultstimelyas outlined. The Transplant Coordinator III-NP alsoassistsother personnel to ensurehigh qualitydelivery of patient care and services to a transplant/end stage organ disease patient population. Develops and coordinates a system to organize the flow of patients frominitialreferral, through evaluation and education, to post-transplant follow-up andreferralback to the community. Participates in collaborative, interdisciplinary research activities related to transplant patients, processimprovementorevidencedbased practice activities. Attend professional meetings and conferences,maintainprofessionalaffiliationsand enhance professional development tomaintainknowledge of advancesoftransplant populations and end-stage organ diseases. Performs other special projects and duties as assigned. This position may require rotating on-call responsibilities as needed, including nights,weekendsand holidays.Essential Duties:

• Furnishes/orders medications, diagnostic studies, and therapeutic procedures related to their illnesses and track resultstimelyas outlined.

• Develops and coordinates a system to organize the flow of patients frominitialreferral, through evaluation and education, to post-transplant follow-up andreferralback to the community.

• Participates in collaborative, interdisciplinary research activities related to transplant patients, processimprovementorevidencedbased practice activities.

• Attend professional meetings and conferences,maintainprofessionalaffiliationsand enhance professional development tomaintainknowledge of advancesoftransplant populations and end-stage organ diseases.

• May beto be on call 24 hours a day and willparticipatein a call rotation, which includes relaying information to physician staff andfacilitatethe ongoing care of the patient as well as handling organ offers.

• Performs other special projects and duties as assigned.

RequiredQualifications:

• ReqMaster'sdegree OR

• Req Doctorate Degree in Nursing Practice Nurse Practitioner Program

• Req 2 years Transplant Coordination experienceand for, Heart Transplant – two years of cardiology experience Lung Transplant – two years of pulmonary experience Liver Transplant – two years of abdominal transplant experience Kidney / Pancreas Transplant – two years of abdominal transplant experience

• Req 2 years Organ Procurement experience and 2 years critical care experience

• Req Must have excellent communication skills, including the ability to speak, read and write English proficiently.

• Req Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures,in accordance withadministrative and clinical policies, regulations and procedures.

Preferred Qualifications:

• Pref 1 year NP experience preferred.

• Req Basic Life Support (BLS) Healthcare Provider from American Heart Association

• Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within30 daysof hire andmaintainedby renewal beforeexpirationdate. (Required within LA City only)

• Req Certification as a Nurse Practitioner through the American Association of Nurse Practitioners or American Nurse Credentialing Center.
