Position Summary:
We have an exciting opportunity to join our team as a Transplant Coordinator, Registered Nurse- Heart Transplant.
In this role, the successful candidate The Heat Failure RN (HF RN) is an important member of the multidisciplinary Heart Failure/Transplant team. The HF RN will oversee care for advanced heart failure patients in the outpatient setting. Specifically, the HF RN is responsible for: collecting clinical information about patients, summarizing medical data, ensuring diagnostic testing and treatment procedures for patients were completed; triaging phone calls from patients; monitoring CardioMEMs Pulmonary Artery Pressure Monitoring system; providing education to patients and families about heart failure symptoms, management, and diet; interfacing with multiple providers to ensure optimal care.
Job Responsibilities:
Return HF Triages: email, phone calls (urgent), In-basket messages
Provides excellent patient services to ensure satisfaction.
Liaisons with the inpatient team to ensure continuous care for the advanced heart failure patients.
Communicates with the multidisciplinary team to optimize patient care. Interfaces with physicians, social workers, nurse practitioners and others.
Maintains contact with assigned caseload between visits, based on their particular needs. Initiates appropriate responses to issues and problems during these contacts.
Ensure laboratory, radiologic, and diagnostic studies scheduled (with FGP medical secretary) and completed.
Educate patients on low salt/high K diet, heart failure symptoms, and management
Complete disability, home health care, FMLA paperwork with FGP medical secretary
Complete paperwork for prior authorizations, denials/appeals with FGP medical secretary
Prepare information for peer to peer authorizations
Keep running list of patients who need follow-up phone calls
Follow-up Heart Failure sign-out
Obtain inpatient CardioMEMS readings
Cardiomems Monitoring: 3x weekly transmission review, weekly notes, weekly phone call, coordinating tech support, review abnormal values with NP, lab follow up, inpatient implant education, preimplant patient education
Simple stress testing
Follow patient labs, tests: alert NP if abnormal, call patient if results normal
Minimum Qualifications:
To qualify you must have a Licensure: Current registration as a Professional Nurse in New York State
Education: Bachelors Degree in Nursing
At least five years of clinical nursing practice, ability to apply nursing process toward achievement of specific outcomes in an organized fashion.
Self-directed worker, able to work independently; excellent interpersonal, communication and organizational skills; creative problem solving and critical thinking ability, understanding of complex antibody profiles.
Previous experience in heart failure patient population. CHFN Certification desirable..
Required Licenses: Registered Nurse License-NYS
