Position Overview
This person will be responsible for patient scheduling and driver scheduling, as well as assist with telephone coordination of the transportation program.
Duties:
· Coordination of weekly/daily schedules & expenditures in specific area.
· Communicate changes in driver/schedule to affected sites when needed.
· Schedule patient transportation
· Collaboration with PHN, PHNCF and out of Network facilities as needed.
· Attend meetings as required.
· Monitor routes for most efficient travel.
· When needed, will drive the patient transportation vehicles. Is responsible for picking up and returning patients to/from scheduled appointments.
· Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the Transportation Department.

Compensation
Position starts at $16.00 per hour, however, PHN recognizes licensure, experience, education and site location when determining wage.
Location and Hours of Position
Work location may vary depending on the employee’s home location within PHN service sites.
Monday – Friday 7:30am – 4:00pm as needed
