Join our team as a Case Manager Registered Nurse at a leading healthcare facility in Eureka, CA. This contract position offers an opportunity to work in a dynamic environment where your skills will make a significant impact on patient care.

Key Responsibilities:

• Manage patient care plans and coordinate services.
• Work collaboratively with healthcare teams to ensure quality care.
• Document patient progress and adjust care plans as needed.
• Provide education and support to patients and their families.

Requirements:

• Active RN license in California.
• Minimum of 2 years of acute care experience.
• First-timers accepted with clinical approval.
• Weekend work required every other weekend.
• Ability to float between like units based on need.
• Charting experience with Epic is preferred.
• All RTO must be included at the time of submittal.
