Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: October 5, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. No floating required.

Highlights:

• 91-day travel assignment at an acute care facility with Epic charting system
• 40 hours of paid orientation to support your successful transition
• Work with interdisciplinary teams including Physical Therapy and Social Services
• Serve diverse patient populations including neonates, adolescents, adults, and geriatrics
• Professional dress code: business attire or scrubs
• Enhanced winter rate period through February 2027

Requirements:

• Active California RN license (pending licenses accepted if active by start date; Compact license must be in hand at submission)
• Minimum 2 years of acute care case management or discharge planning experience required
• Hospital experience mandatory; behavioral health experience does not substitute
• Seasoned travel experience required; first-time travelers cannot be considered
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Proficiency with HMOs, IPAs, and knowledge of foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, and protective services
• Competent with independently arranging DME, SNF placements, and ambulance transport
• Ability to organize and participate in family meetings and treatment plan development
• Two professional references covering one year within the last three years
• Clean background check required; no exceptions for background discrepancies
• Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
