Job Summary
Coordinates patients through the acute care hospitalization process, managing all aspects of discharge planning and transition. Performs utilization management and quality screenings. In-patient case management experience is required.

Required Qualifications

• BSN required
• RN license required

• CCMC certification preferred

• 2 years of acute care experience strongly preferred
• Experience in case management and managed care
