American Consultants is seeking a travel nurse RN Case Management for a travel nursing job in Apple Valley, California.

Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: August 24, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), with every other weekend rotation to meet department needs. Patient ratios: 1:25 on weekdays, 1:45 on weekends. 40 hours of orientation provided.

Highlights:

  • 13-week travel assignment at an acute care facility
  • Work with diverse patient populations including neonates, adolescents, adults, and geriatrics
  • Collaborate with interdisciplinary support teams including physical therapy and social services
  • Caseload of up to 25 patients with focus on discharge planning and care coordination
  • Business attire or scrubs accepted
  • Epic charting system experience required

Requirements:

  • Active California RN license (pending license accepted but must be active by start date)
  • Minimum 2 years of acute care case management or discharge planning experience required
  • Hospital experience mandatory; behavioral health experience does not substitute
  • Previous travel nursing experience required – seasoned travelers only
  • Epic charting system proficiency required
  • Experience with assessing patients, care coordination, discharge planning, and utilization review
  • Knowledge of HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness resources, mental health crisis intervention, advance directives, and protective services
  • Competent with arranging DME, SNF placement, and ambulance transport independently
  • Ability to organize and participate in family meetings and treatment plan development
  • Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
  • Two professional references covering 1 year within the last 3 years
