Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: October 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Weekend rotation required (every other weekend). Patient ratios: 1:25 on weekdays, 1:45 on weekends. No floating required.

Requirements:

• Active California RN license required (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting required
• Previous travel nursing experience required; seasoned travelers preferred
• Hospital experience mandatory; behavioral health experience does not substitute for acute care requirements
• Experience with patient assessment, care coordination, discharge planning, and utilization review
• Proficiency with HMOs, IPAs, DRGs, CMS guidelines, and Joint Commission standards
• Competent with arranging DME, SNF placements, and ambulance transport independently
• Knowledge of community resources including foster care, substance abuse treatment, homelessness services, and mental health crisis intervention
• Two professional references covering one year within the last three years required
• Clean background check required; no exceptions
