Your Role

As a Case Manager, you will provide comprehensive care coordination and discharge planning for acute-care patients. You will assess patient needs, coordinate services, support safe transitions of care, and ensure appropriate utilization of healthcare resources.

Key Responsibilities

  • Perform comprehensive patient assessments and care coordination.
  • Develop and manage individualized plans of care.
  • Perform discharge planning and coordinate safe transitions of care.
  • Determine medical necessity using evidence-based guidelines.
  • Coordinate DME and other post-acute services based on patient needs.
  • Support disease management and utilization review activities.
  • Work with HMOs, IPAs, and other healthcare partners.
  • Review admission criteria and support appropriate level-of-care decisions.
  • Maintain accurate computerized documentation in Epic.
  • Apply CMS, HIPAA, DRG, CPT, and Department of Health requirements.
  • Collaborate with interdisciplinary teams to support patient outcomes.

Qualifications / Required

  • 2+ years of acute medical Care Manager experience performing care coordination and discharge planning.
  • RN experience in an acute hospital setting.
  • Epic experience required.
  • Experience with case management/utilization review.
  • Strong care coordination and discharge planning experience.
  • Experience assessing patient needs and determining appropriate services.
  • Knowledge of medical necessity and evidence-based guidelines.
  • Experience with ICU, Med-Surg, SDU/PCU/IMC/Observation populations preferred.
  • California RN license required; pending license may be considered but must be active by start date.
  • Travel experience required; first-time travelers may be screened.
  • Every other weekend requirement, with a weekday off when working the weekend.
  • Up to 7 days of RTO allowed.

Ideal Candidate

  • Experienced acute-care Case Manager with strong care coordination and discharge planning skills.
  • Comfortable managing a caseload of up to 25 patients.
  • Experienced with Epic and computerized clinical documentation.
  • Knowledgeable in utilization review, medical necessity, regulatory requirements, and post-acute planning.
  • Able to work independently while collaborating effectively with physicians and interdisciplinary teams.
