Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: November 2, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), every other weekend rotation required. Patient ratios: 1:25 weekdays, 1:45 weekends. No floating required.

Requirements:

• Active California RN license required (pending licenses accepted if active by start date)
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
• Previous travel nursing experience required; seasoned travelers preferred
• Hospital-based experience mandatory; behavioral health-only experience does not qualify
• Experience with care coordination, utilization review, discharge planning, and patient assessment
• Competency in arranging DME, SNF placements, and ambulance transport independently
• Knowledge of HMOs, IPAs, CMS guidelines, DRGs, and regulatory compliance
• Ability to organize and participate in family meetings and treatment plan development
• Familiarity with special populations including foster care, substance abuse treatment, homelessness, and mental health crisis intervention
• Two professional references covering one year within the last three years required
