Position Overview
We are seeking an experienced RN Case Manager to join an acute care hospital team. The Case Manager will assess patient needs, coordinate care, develop and implement treatment plans, facilitate discharge planning, and collaborate with multidisciplinary teams to support quality patient outcomes.
The ideal candidate will have strong acute care hospital case management and discharge planning experience and be comfortable working independently in a fast-paced hospital environment.
Key Responsibilities
- Perform comprehensive patient assessments and needs assessments.
- Coordinate patient care across interdisciplinary teams.
- Develop and participate in individualized treatment plans.
- Conduct discharge planning and facilitate safe transitions of care.
- Perform utilization review and admission criteria assessments.
- Coordinate DME, SNF placement, ambulance transportation, and other post-acute services.
- Participate in family meetings and collaborate with patients, families, physicians, and care teams.
- Work with HMOs, IPAs, and other healthcare organizations.
- Assist with prior authorizations and benefits eligibility.
- Apply knowledge of CMS requirements, DRGs, Core Measures, and National Patient Safety Goals.
- Support patients with complex social and healthcare needs.
- Maintain accurate computerized documentation.
Required Qualifications
- Active California RN license required.
- Pending California license may be accepted, but the license must be active by the start date.
- Minimum 2 years of acute care case management/discharge planning experience.
- Recent hospital experience is required.
- Travel nursing experience required.
- Strong experience with care coordination, discharge planning, and utilization review.
- Ability to independently coordinate DME, SNF placement, and transportation.
- Strong communication, organizational, and clinical assessment skills.
Behavioral health experience alone does not meet the hospital experience requirement.

Additional Requirements
- Must be a seasoned traveler; first-time travelers are not accepted.
- Must commit to working 3 of 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, or New Year's Day.
- All requested time off must be submitted upfront.
- Maximum RTO: 7 days.
- Travelers may be required to float to nearby sister facilities.
- Computerized documentation experience required.
- BLS certification required.
- Epic experience is preferred.
