Start Date: October 19, 2026

Location: Mission Viejo, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). Variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be consecutive, although every effort will be made to schedule consecutive days off when possible. Must commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).

• Patient ratio of 1:25 with focus on care coordination and discharge planning
• 40 hours of paid orientation to support your transition
• May float to nearby sister facilities within the health system
• Dress code: Turquoise scrubs or business casual
• Epic charting system experience required

Requirements:

• Active California RN license (pending license accepted but must be active by start date)
• Minimum 2 years of acute medical Care Manager experience performing care coordination
• Previous travel nursing experience required
• Epic charting system proficiency (hard requirement)
• Experience with patient assessment, care coordination, discharge planning, and family meetings
• Knowledge of HMOs, IPAs, DME arrangements, SNF placement, and ambulance transport coordination
• Familiarity with adoption/foster care, substance abuse treatment options, homelessness resources, mental health crisis management, advance directives, and POLST
• Competent with appeals and denials, DRG coding, CMS guidelines, HIPAA, and Joint Commission standards
• Two professional references covering 1 year within the last 3 years
